Thank you for your interest in Saint Vincent de Paul School!  
 
The following documents are required to complete your application:
  • Child's Birth Certificate
  • Child's Baptism Certificate (if applicable) 
  • Child's Immunization records
  • A recent family photograph  
These documents may be scanned and sent to the Admissions Director via email at admissions@svdpsf.com, Uploaded via DocuSign, or mailed to:

 SVDP also requires School Recommendation Forms for all students plus transcripts and test scores for transfer students.  We will follow up with you regarding these forms once your application and supporting documents are submitted.
